Emergency contact form:

This includes all addresses, and phone numbers of doctors, hospitals, emergency contacts. There needs to be two (2) emergency contact people. Grandparents or other relatives at the same address can count as one contact.

Registration Fees:

Carebear Preschool requires a registration fee of $150.00 and first and last month's tuition is due at the time of enrollment. This NON-REFUNDABLE and non transferable.  Disenrollment of a child requires a 30-day written notice submitted to the Director.

Immunizations:

It is the state of Arizona’s policy that your child receives immunizations in order to attend school. Therefore, it is extremely important that your child’s immunization records be kept up to date. It is the parent’s responsibility to bring a copy of their child’s immunization records and update them for Carebear Preschool’s files.

All records kept on your child are confidential. Your child’s records include enrollment information, medical information, accident reports, and assessment/progress reports. You have a right to view your child’s records. We ask that you call the school office in advance if you would like to see these records.

FAILURE TO ACCURATELY COMPLETE FORMS OR PAY FEES IN A TIMELY MANNER MAY RESULT IN CANCELLATION OF THE ENROLLMENT AGREEMENT.
